Estudio comparativo entre cintas e hilos para la sutura del manguito rotador en doble hilera

Resumen

Introducción La sutura de manguito puede realizarse con o sin nudos, con riesgo de rerrotura mayor con nudos, p=0.00094. La utilización de cintas permitiría distribuir la compresión en una superficie mayor, previniendo la rerrotura. Nuestro objetivo fue valorar nuestra experiencia en la sutura de manguito, al comparar la doble hilera, con hilos y nudos mediales, frente a la doble hilera, con cintas, y sin nudos. Material y métodos Estudio longitudinal, prospectivo, analítico, observacional de cohortes. Nivel de evidencia II. Comparativo entre dos tipos de sutura en doble hilera: hilos con nudos, frente a cintas sin nudos. Variables: datos de filiación, cirugía, rehabilitación, Constant, E.V.A. y complicaciones. Resultados 57 pacientes. Edad: 52,7 +/- 6,4 años. Tiempo de cirugía: 91,4 +/- 23,2 minutos. Rehabilitación: 4,1 +/- 1 meses. Constant final: 72.9 +/- 15,6 puntos. E.V.A. final: 1. El tiempo de rehabilitación, rigidez y reintervención fue menor con cintas, (p=0.02), (22.6% Vs 42.3%, p=0.9), (6.5% Vs 15.4%, p=0.39), respectivamente, alcanzando mayor reincorporación laboral (61.3% Vs 37.55, p=0.14). Concluisiones La sutura en doble hilera (cintas sin nudos), supondría menor tiempo de rehabilitación, rigidez y necesidad de reintervención que cuando se usaban hilos anudados, con anterior retorno al nivel funcional previo, sin influencia en Constant.
